Hi Mi,
I fully understand what you mean. There is a gradual change from normal smooth muscle cells to neoplastic cells, and leiomyomas will contain cells that are almost similar to the normal cells. In the section it is of course not possible to see the mutations required for the growth of a neoplasm. In the specimen, the leiomyoma has shrunk a little, and therefore what you see with surrounding “empty space” in the central part of the specimen is the tumour.
